REQUIREMENTS FOR NEW NAMIBIAN PASSPORTS

Original documents must be submitted together with passport application form
1. Namibian Birth Certificate or (Citizenship Certificate-persons born outside Namibia)
2. Namibian ID-card(From 16 years old)
3. Marriage Certificate (Married women only)
4. One biometric passport photo with your name at the back
5. Present Passport
6. Fees: € 40.00 (New passport)
? 100.00 (For damaged or lost passport)

CHILDREN UNDER AGE 0-15
Original documents must be submitted together with passport application form
1. Birth Certificate or (Citizenship Certificate-persons born outside Namibia)
2. One biometric passport photo with the child’s name written on the back of the photo
3. Parents Documents e.g. Birth or Citizenship Certificates or ID
4. Applicant’s signature (14 years old and above)
5. Present Passport
6. Fees: € 40.00 (New passport)
? 100.00 (For damaged or lost passport)
7. Applicant?s signature (14 years old and above)

REQUIREMENTS FOR EMERGENCY TRAVEL CERTIFICATE
Original documents must be submitted together with passport application form
1. Namibian Birth Certificate or (Citizenship Certificate-persons born outside Namibia)
2. Two biometric passport photos with your name written on the back of the photo
3. Copy of ID-card or proof of ID-card (16 years and above)
4. Marriage Certificate (married women only)
5. Parents Documents e.g. Birth or Citizenship Certificates or ID(applicants under the age of
16)
6. Fees: € 30.00
7. Applicant?s signature (14 years old and above)

Loss of Documents

ID CARDS

1. Application for Duplicate Forms are available at the Embassy
2. Fingerprints (taken at the Embassy)

BIRTH CERTIFICATE

1. Application for Duplicate / Full Birth Certificate (Form)
2. Abridged Birth Certificate (if applicable)
3. Copy of ID(if applicable)

?Marriage Certificate

1. Duplicate Form
2. Identification (Original and certified copy of Birth Certificate or ID)
3. ID of applicant
   4. Affidavit stating what happened to the original document
   5. Fee – 20€
